Take the Orange Line to Forest Hills.
Take the 36 Charles River bus to Belgrade Avenue and West Roxbury Parkway. The 36 bus departs from the Forest Hills Station Upper Busway every half hour.
Walk north on West Roxbury Parkway to Centre Street. Holy Name is a 5 minute walk from the West Roxbury Parkway bus stop.

By commuter rail
Take the Needham Line to Bellevue. The Neeham Line departs from South Station, Back Bay, and Forest Hills, approximately once an hour. See the schedule here.
From the Bellevue station, walk west, take a right onto West Roxbury Parkway, take a right onto Centre Street. Holy Name is a 5 minute walk from the Bellevue station.
